The CCHP Designation is a professional and official Canadian exam. These policies and forms protect the candidate’s information and exam content

The following policies and forms each have a very specific purpose to inform and prepare candidates for their registration and exam. All applicable forms must be completed and submitted to the Registrar’s Office before your exam registration can be considered. Please find the registrar’s contact information below:

The Work Experience Portfolio is necessary in proving one’s eligibility for this exam, as it contains all relevant HACCP work, academic, or volunteer experience. Without this form submitted to the registrar, the candidate will not be accepted.

All candidates must complete and submit a Work Experience Portfolio to the Registrar to begin the CCHP application process.

To protect the candidate, FPHRC, and the exam content, please complete, sign, and submit the Statement of Accountability, Ethics, and Confidentiality with the Work Experience Portfolio. This form ensures the stated experience and contact information is correct, and that no exam content or information will be shared outside of the writing and registration process.

If you have a disability or other special needs that could interfere with your test performance, you will need to complete the accommodation request form.

Accommodation (Special Testing) Request Form

Candidates with a disability or additional needs may request special accommodations by completing and submitting this form. It is requested that candidates submit their special accommodations request as soon as possible; at latest, a minimum of 5 business days before your scheduled exam. Candidates that have submitted a request will be contacted by the Registrar’s office within a reasonable timeframe, indicating a decision. Special accommodation requests will be assessed on a case-by-case basis and are not guaranteed.

Special testing requests will be reviewed to determine if appropriate assistance is available. A special testing request will only be granted when there is reasonable assurance that the candidate will have a fair and equitable opportunity to write the exam. Special testing measures must be reasonable while not compromising the validity and reliability of the assessment instruments.

RELATED TO LANGUAGE FORMAT

The CCHP exam is available in either English or French versions. Individuals that are not proficient in either language may request special testing, however, it is important to note that all exams are based on the Occupational Standard for the profession, and reflect the type and level of language contained in the Standard. Special testing measures will not overcome issues associated with literacy or lack of ability to communicate on the job.

Please review the CCHP Professional Code of Ethics. Recipients of this designation will uphold a high level of integrity in the food processing industry, and must complete their duties in an ethical and responsible manner.

Candidates who fail the CCHP Designation exam have the right to register an appeal on the procedures used and the results of their examination. Appeals must be submitted in writing to the Registrar within 60 days of receiving the exam results.

The cost of an Appeal is $50CDN.

Every three years, your CCHP designation will need to be renewed. Failure to meet the requirements and submit this application after four years will result in the individual’s status being changed to “inactive”.
